ABOUT THE SPEAKER
David Agus - Cancer Doctor
Although a highly-accomplished conventional doctor, David Agus has embraced the future of medicine and is constantly exploring ways that new technologies can help in the fight against cancer.

Why you should listen

David Agus is a medical doctor and a Professor of Medicine at the University of Southern California. However, he is also the founder of a couple of game-changing medical initiatives. In 2006, he co-founded Navigenics with Dietrich Stephan, Ph.D., to form a company that would provide people with their individual genetic information, allowing them to act on any predispositions to disease that they might have and prevent onset. He also founded Oncology.com which was the largest cancer Internet resource and community.

Dr. Agus’ research is focused on the application of proteomics and genomics in the study of cancer, as well as developing new therapeutic treatments for cancer. He serves as Director of the USC Center for Applied Molecular Medicine and the USC Westside Prostate Cancer Center. Agus is also the recipient of several honors and awards, including the American Cancer Society Physician Research Award, a Clinical Scholar Award from the Sloan-Kettering Institute and the International Myeloma Foundation Visionary Science Award.

58
183000

03:21
Because the problem문제 is, when you have advanced많은 cancer,
59
186000
3000
03:24
we can't do that much today오늘 about it, as the statistics통계 allude언급하다 to.
60
189000
4000
03:28
So, the thing about cancer is that it's a disease질병 of the aged늙은.
61
193000
4000
03:32
Why is it a disease질병 of the aged늙은?
62
197000
2000
03:34
Because evolution진화 doesn't care케어 about us after we've우리는 had our children어린이.
63
199000
4000
03:39
See, evolution진화 protected보호 된 us during...동안 our childbearing분만 years연령
64
204000
3000
03:42
and then, after age나이 35 or 40 or 45,
65

03:46
it said "It doesn't matter문제 anymore더 이상, because they've그들은 had their그들의 progeny자손."
66
211000
4000
03:50
So if you look at cancers, it is very rare드문 -- extremely매우 rare드문 --
67
215000
5000

04:00
As one gets도착 older더 오래된? Very, very common공유지.
69
225000
4000
04:04
Why is it hard단단한 to treat?
70
229000
2000
04:06
Because it's heterogeneous
71
231000
2000
04:08
and that's the perfect완전한 substrate기판 for evolution진화 within이내에 the cancer.
72
233000
5000
04:13
It starts시작하다 to select고르다 out for those bad나쁜, aggressive적극적인 cells세포들,

04:17
what we call clonal클론 selection선택.
74
242000
4000
04:21
But, if we start스타트 to understand알다
75
246000
3000
04:24
that cancer isn't just a molecular분자 defect결함, it's something more,
76
249000
5000
04:29
then we'll get to new새로운 ways of treating치료 it, as I'll show보여 주다 you.
77
254000
4000
04:33
So, one of the fundamental기본적인 problems문제들 we have in cancer
78
258000
2000
04:35
is that, right now, we describe기술하다 it by a number번호 of adjectives형용사, symptoms조짐:
79
260000
4000
04:39
"I'm tired피곤한, I'm bloated부푼, I have pain고통, etc기타."
80
264000
3000
04:42
You then have some anatomic해부학의 descriptions기술,
81
267000
2000
04:44
you get that CTCT scan주사: "There's a three centimeter센티미터 mass질량 in the liver."
82
269000
4000
04:48
You then have some body신체 part부품 descriptions기술:
83
273000
3000
04:51
"It's in the liver, in the breast유방, in the prostate전립선."
84
276000
2000
04:53
And that's about it.
85
278000
3000
04:56
So, our dictionary사전 for describing묘사하는 cancer is very, very poor가난한.
86
281000
4000
05:00
It's basically원래 symptoms조짐.
87
285000
2000
05:02
It's manifestations현시 of a disease질병.
88
287000
3000
05:05
What's exciting흥미 진진한 is that over the last two or three years연령,
89
290000
3000
05:08
the government정부 has spent지출하다 400 million백만 dollars불화,
90
293000

92
298000
2000
05:15
So, it is the idea생각 of sequencing시퀀싱 all of the genes유전자 in the cancer,
93
300000
4000
05:19
and giving주는 us a new새로운 lexicon사전, a new새로운 dictionary사전 to describe기술하다 it.
94
304000
5000
05:24
You know, in the mid-중간 -1850's'에스 in France프랑스,
95
309000
3000
05:27
they started시작한 to describe기술하다 cancer by body신체 part부품.

05:30

97
315000

05:34
It is absolutely전혀 archaic고풍의 that we call cancer

05:42
It makes~을 만든다 no sense감각, if you think about it.
100
327000
3000

05:45
So, obviously명백하게, the technology과학 기술 is here today오늘,

So, one of the problems문제들, the fundamental기본적인 problems문제들 in medicine의학
189
614000

10:32

You know, I've been using~을 사용하여 it for two and a half절반 months개월.

10:46
It's a staggering비틀 거리는 device장치, not because it tells말하다 me

10:48
how many많은 kilocalories킬로 칼로리 I do every...마다 day,
196
633000

10:51
but because it looks외모, over 24 hours시간, what I've done끝난 in a day.
197
636000

10:55
And I didn't realize깨닫다 that for three hours시간 I'm sitting좌석 at my desk책상,
198
640000

10:58
and I'm not moving움직이는 at all.
199
643000
2000
11:00
And a lot of the functions기능들 in the data데이터 that we have as input입력 systems시스템 here
200
645000
5000
11:05
are really different다른 than we understand알다 them,
201
650000

11:08
because we're not measuring자질 them dynamically역동적 인.
202
653000
2000
11:10
And so, if you think of cancer as a system체계,
203
655000
5000
11:15
there's an input입력 and an output산출 and a state상태 in the middle중간.
204
660000
4000
11:19
So, the states, are equivalent동등한 classes수업 of history역사,
205

11:22
and the cancer patient환자, the input입력, is the environment환경,
206

11:29
The output산출 are our symptoms조짐:
208
674000
3000
11:32
Do we have pain고통? Is the cancer growing성장하는? Do we feel bloated부푼, etc기타.?
209
677000
4000
11:36
Most가장 of that state상태 is hidden숨겨진.
210
681000
4000
11:40
So what we do in our field is we change변화 and input입력,
211
685000
3000
11:43
we give aggressive적극적인 chemotherapy화학 요법,
212
688000
2000
11:45
and we say, "Did that output산출 get better? Did that pain고통 improve돌리다, etc기타.?"
213
690000
5000
11:50
And so, the problem문제 is that it's not just one system체계,
214
695000
4000
11:54
it's multiple배수 systems시스템 on multiple배수 scales저울.
215

It's a system체계 of systems시스템.
216

The problem문제 is that, if I'm this doc의사, and I get your CTCT scan주사 today오늘

So, it's a fundamental기본적인 problem문제.

The way we've우리는 approached접근하다 it is the field of proteomics프로테오믹스,

It's taking취득 a big picture그림.
380
1221000

20:38
The problem문제 with technologies기술 like this is
381
1223000
3000
20:41
that if one looks외모 at proteins단백질 in the body신체,

384
