00:49
I'm in the operating操作 room房间. We have Bob短发 here,

00:57
So actually其实 how do we harvest收成 this bone marrow骨髓?
15
39000
2000
00:59
Well we have a whole整个 O.R. team球队, general一般 anesthesia麻醉, nurses护士,
16
41000
4000
01:03
and another另一个 doctor医生 across横过 from me.
17
45000
2000
01:05
Bob's鲍勃的 on the table, and we take this sort分类 of small needle,
18
47000
2000
01:07
you know, not too big.
19
49000
2000
01:09
And the way we do this is we basically基本上
20
51000
2000
01:11
place地点 this through通过 the soft柔软的 tissue组织,
21
53000
2000
01:13
and kind of punch冲床 it into the hard bone,
22
55000
2000
01:15
into the tuchustuchus -- that's a technical技术 term术语 --
23
57000
2000
01:17
and aspirate吸气 about 10 mlsMLS of bone marrow骨髓 out,
24
59000
4000
01:21
each time, with a syringe注射器.
25
63000
2000
01:23
And hand it off to the nurse护士. She squirts it into a tin.
26
65000
3000
01:26
Hands it back to me. And we do that again and again.
27
68000
3000
01:29
About 200 times usually平时.
28
71000
2000
01:31
And by the end结束 of this my arm is sore, I've got a callus胼胝 on my hand,
29
73000
2000
01:35
whose谁的 rear end结束 looks容貌 something more like this,
31
77000
2000
01:39
So I'm thinking思维, you know, this procedure程序 hasn't有没有 changed in about 40 years年份.
33
81000
4000
01:43
And there is probably大概 a better way to do this.
34
85000
2000
01:45
So I thought of a minimally微创 invasive侵入的 approach途径,
35
87000
3000
01:48
and a new device设备 that we call the Marrow骨髓 Miner矿工.
36
90000
2000
01:50
This is it.
37
92000
2000
01:52
And the Marrow骨髓 Miner矿工, the way it works作品 is shown显示 here.
38
94000
3000
01:55
Our standard标准 see-through透视 patient患者.
39
97000
2000
01:57
Instead代替 of entering进入 the bone dozens许多 of times,
40
99000
2000
01:59
we enter输入 just once一旦, into the front面前 of the hip臀部 or the back of the hip臀部.
41
101000
2000
02:01
And we have a flexible灵活, powered动力 catheter导管
42
103000
3000
02:04
with a special特别 wire线 loop循环 tip小费 that stays入住 inside the crunchy脆脆 part部分 of the marrow骨髓
43
106000
3000
02:07
and follows如下 the contours轮廓 of the hip臀部, as it moves移动 around.
44
109000
3000
02:10
So it enables使 you to very rapidly急速 aspirate吸气,
45
112000
2000
02:12
or suck吮吸 out, rich丰富 bone marrow骨髓 very quickly很快 through通过 one hole.
46
114000
3000
02:15
We can do multiple passes通行证 through通过 that same相同 entry条目.
47
117000
2000
02:19
And, so, very quickly很快, Bob短发 can just get one puncture, local本地 anesthesia麻醉,
49
121000
3000
02:22
and do this harvest收成 as an outpatient门诊病人.
50
124000
4000
02:26
So I did a few少数 prototypes原型. I got a small little grant发放 at Stanford斯坦福.
51
128000
3000
02:29
And played发挥 around with this a little bit.
52
131000
2000
02:31
And our team球队 members会员 developed发达 this technology技术.
53
133000
2000
02:33
And eventually终于 we got two large animals动物, and pig studies学习.
54
135000
4000
02:37
And we found发现, to our surprise, that we not only got bone marrow骨髓 out,
55
139000
2000
02:39
but we got 10 times the stem cell细胞 activity活动
56
141000
3000
02:42
in the marrow骨髓 from the Marrow骨髓 Miner矿工, compared相比 to the normal正常 device设备.
57
144000
2000
02:44
This device设备 was just FDAFDA approved批准 in the last year.
58
146000
3000
02:47
Here is a live生活 patient患者. You can see it following以下 the flexible灵活 curves曲线 around.
59
149000
3000
02:50
There will be two passes通行证 here, in the same相同 patient患者, from the same相同 hole.
60
152000
3000
02:53
This was doneDONE under local本地 anesthesia麻醉, as an outpatient门诊病人.
61
155000
2000
02:55
And we got, again, about three to six times more stem cells细胞
62
157000
3000
02:58
than the standard标准 approach途径 doneDONE on the same相同 patient患者.
63
160000
3000
03:01
So why should you care关心? Bone marrow骨髓 is a very rich丰富 source资源 of adult成人 stem cells细胞.
64
163000
3000

You all know about embryonic胚胎 stem cells细胞.
