ABOUT THE SPEAKER
Sandeep Jauhar - Physician, writer
Sandeep Jauhar is a practicing cardiologist passionate about communicating medicine in all its glorious, quirky, inescapable humanity.

Why you should listen

In addition to his medical practice, Dr. Sandeep Jauhar writes about medicine and its impacts on culture for a wide-ranging audience. He is a contributing opinion writer for the New York Times and has written three best-selling books.

His most recent book, Heart: A History (a finalist for the 2019 Wellcome Book prize), tells the little-known story of the doctors who risked their careers -- and the patients who risked their lives -- to understand our most vital organ. Weaving his own experiences with the defining discoveries of the past, Jauhar braids tales of breakthrough, hubris and sorrow to create a lucid chronicle of our life's most intimate chamber. The book also confronts the limits of medical technology, arguing that future progress will be determined more by how we choose to live rather than by any device we invent. Indeed, as the book explains, our emotional lives are absolutely key to our heart health. Jauhar is the Director of the Heart Failure Program at Northwell Health's Long Island Jewish Medical Center.

"Um registro da nossa vida emocional está inscrito em nosso coração", diz o cardiologista e autor Sandeep Jauhar. Nesta magnífica palestra, ele explora as formas misteriosas pelas quais nossas emoções impactam a saúde do nosso coração, levando-o a mudar de formato em resposta à angústia ou ao medo e a literalmente se partir em resposta à angústia emocional, e clama por uma reorientação na forma de cuidarmos desse nosso órgão essencial.

Nenhum outro órgão,

perhaps no other object in human life,

3653
talvez nenhum outro objeto na vida humana,
00:18
is as imbued with metaphor
and meaning as the human heart.
2

está tão imbuído de metáforas
e significados quanto o coração humano.
00:23
Over the course of history,
3
11347
1411
Ao longo da história,
00:24
the heart has been a symbol
of our emotional lives.
4
12782
3341
o coração tem sido o símbolo
da nossa vida emocional,
00:28
It was considered by many
to be the seat of the soul,
5
16520
4302
considerado por muitos o centro da alma,
00:32
the repository of the emotions.
6

o repositório das emoções.
00:35
The very word "emotion" stems in part
from the French verb "émouvoir,"
7
23278
6774
A própria palavra "emoção" se origina
em parte do verbo francês "émouvoir",
00:42
meaning "to stir up."
8
30076
1546
que significa "agitar".
00:44
And perhaps it's only logical
that emotions would be linked to an organ
9
32028
4488
E talvez seja lógico que as emoções
estejam ligadas a um órgão
00:48
characterized by its agitated movement.
10
36540
2947
caracterizado por seu movimento agitado.
00:51
But what is this link?
11
39511
1559
Mas qual é essa ligação?
00:53
Is it real or purely metaphorical?
12
41492
3124
É real ou meramente metafórica?
00:57
As a heart specialist,
13
45426
1971
Como cardiologista,
00:59
I am here today to tell you
that this link is very real.
14
47421
5537
estou aqui hoje para dizer a vocês
que essa ligação é real.
01:05
Emotions, you will learn,
15
53391
2074
As emoções, como veremos,
01:07
can and do have a direct
physical effect on the human heart.
16
55489
5417
podem ter e têm um efeito físico
direto no coração humano.
Mas antes de entrar no assunto,
01:14
But before we get into this,
17
62446
1376
01:15
let's talk a bit about
the metaphorical heart.
18
63846
2463
vou falar um pouco
sobre o coração metafórico.
01:18
The symbolism of the emotional heart
endures even today.
19
66879
4057
O simbolismo do coração emocional
perdura até hoje.
01:23
If we ask people which image
they most associate with love,
20
71415
5222
Se perguntarmos a uma pessoa
que imagem ela mais associa ao amor,
01:28
there's no question that the Valentine
heart would the top the list.
21
76661
4320
sem dúvida alguma o símbolo do coração
estaria no topo da lista.
01:33
The heart shape, called a cardioid,

A forma do coração, chamada de cardioide,
01:36
is common in nature.
23

01:38
It's found in the leaves,

which was used for birth control
in the Middle Ages
26

3234
que era usado como contraceptivo
durante a Idade Média
01:48
and perhaps is the reason why
the heart became associated
27
96711
3321
e talvez seja por isso
que o coração ficou associado
01:52
with sex and romantic love.
28
100056
2675
ao sexo e ao amor romântico.
01:55
Whatever the reason,
29
103708
1384
Qualquer que seja a razão,
01:57
hearts began to appear in paintings
of lovers in the 13th century.
30

In the Roman Catholic Church,

02:11
the heart shape became known
as the Sacred Heart of Jesus.
35

02:24
This association between the heart

tem perdurado na modernidade.

supostamente perguntou aos médicos:
02:45
"Will he still be able to love me?"
42
153780
1984
"Ele ainda será capaz de me amar?"
02:48
Today, we know that the heart
is not the source of love
43
156715
3460
Atualmente, sabemos que o coração
não é a fonte do amor
02:52
or the other emotions, per se;
44
160199
2004
ou de outras emoções, necessariamente;
02:54
the ancients were mistaken.
45
162227
1656
os antigos estavam enganados.
02:55
And yet, more and more,
we have come to understand
46
163907
2587
Entretanto, cada vez mais
começamos a entender
02:58
that the connection between the heart
and the emotions is a highly intimate one.
47
166518
4801
que a conexão entre o coração
e as emoções é extremamente íntima.
03:03
The heart may not originate our feelings,
48
171343
2663
O coração pode não ser a fonte
dos nossos sentimentos,
03:06
but it is highly responsive to them.
49

2094
mas é intensamente sensível a eles.
03:08
In a sense, a record of our emotional life
50
176148
2902
De certo modo, um registro
da nossa vida emocional
03:11
is written on our hearts.
51
179074
2138
está inscrito em nosso coração.
03:13
Fear and grief, for example,
can cause profound cardiac injury.

O medo e a tristeza, por exemplo,
podem causar profundo dano cardíaco.
03:18
The nerves that control unconscious

Os nervos que controlam processos
inconscientes, como o batimento cardíaco,
03:23
can sense distress
54
191210
1822
podem sentir a aflição
03:25
and trigger a maladaptive
fight-or-flight response
55
193056
4938
e desencadear uma reação
inadequada de fuga ou luta
03:30
that triggers blood vessels to constrict,
56
198018
3488
que faz os vasos sanguíneos se retraírem,
03:33
the heart to gallop
57
201530
1730

03:35

58
203284

e a pressão sanguínea subir,
03:37
resulting in damage.
59
205889
1754
causando danos.
03:39
In other words,
60
207667
1761
Em outras palavras,
03:41
it is increasingly clear
61
209452
2640
está cada vez mais claro
03:44
that our hearts are extraordinarily
sensitive to our emotional system,
62
212116
5046
que nosso coração é extraordinariamente
sensível ao nosso sistema emocional,
03:49
to the metaphorical heart, if you will.
63
217186
3262
ou, se preferirem,
ao nosso coração metafórico.
03:52
There is a heart disorder
first recognized about two decades ago
64
220472
4448
Há uma doença cardíaca reconhecida
pela primeira vez há duas décadas
03:56
called "takotsubo cardiomyopathy,"
or "the broken heart syndrome,"
65
224944
4922
chamada "cardiomiopatia de takotsubo",
ou "síndrome do coração partido",
04:01
in which the heart acutely weakens
in response to intense stress or grief,
66
229890
6092
na qual o coração se enfraquece gravemente
em reação a forte estresse ou sofrimento,
04:08
such as after a romantic breakup
or the death of a loved one.
67
236006
3882
como o fim de um relacionamento
ou a morte de um ente querido.
04:11
As these pictures show,
the grieving heart in the middle
68
239912
3691
Como mostram essas fotos,
o coração que sofre, no meio,
04:15
looks very different
than the normal heart on the left.
69
243627
3196
parece bastante diferente
do coração normal, à esquerda.
04:18
It appears stunned
70
246847
1309
Ele parece atordoado
04:20
and frequently balloons into
the distinctive shape of a takotsubo,
71
248180
4416
e frequentemente infla até se parecer
distintamente com um takotsubo,
04:24

and a narrow neck.
73

04:29
We don't know exactly why this happens,
74
257981
2884
Não sabemos exatamente
por que isso acontece
04:32
and the syndrome usually resolves
within a few weeks.
75
260889
2602
e a síndrome costuma se corrigir
em algumas semanas.
04:35
However, in the acute period,
76
263515
1974
Entretanto, no período agudo,
04:37
it can cause heart failure,
77
265513
2773
ela pode causar insuficiência cardíaca,
04:40
life-threatening arrhythmias,
78
268310
1957
arritmias fatais,
04:42
even death.
79
270291
1363
até mesmo morte.
04:43
For example, the husband
of an elderly patient of mine
80
271678
5408
Por exemplo, o marido
de minha paciente idosa
04:49
had died recently.
81
277110
1553
morreu recentemente.
04:51
She was sad, of course, but accepting.
82
279362
3321
Ela estava triste, claro, mas conformada.
04:55
Maybe even a bit relieved.
83
283568
1880
Talvez até um pouco aliviada.
04:57
It had been a very long illness;
he'd had dementia.
84
285472
2618
Havia sido uma doença muito longa;
ele tinha demência.
05:00
But a week after the funeral,
she looked at his picture
85
288114
3565
Mas, uma semana após o funeral,
ela olhou para uma foto
05:04
and became tearful.
86
292481
1389
e se sentiu triste.
05:06
And then she developed chest pain,
and with it, came shortness of breath,
87
294811
5074
E então ela desenvolveu dor no peito,
acompanhada de falta de ar,
05:11
distended neck veins, a sweaty brow,
88
299909
3006
veias do pescoço dilatadas e suor na testa
05:14
a noticeable panting
as she was sitting up in a chair --
89
302939
3342
e ofegava de forma perceptível
ao se sentar em uma cadeira;
05:18
all signs of heart failure.
90
306305
3986
todos eles sinais
de insuficiência cardíaca.
05:23
She was admitted to the hospital,
91
311270
2224
Ela deu entrada no hospital,
05:25
where an ultrasound confirmed
what we already suspected:
92
313518
4071
onde uma ultrassonografia
confirmou o que já suspeitávamos:
05:29
her heart had weakened
to less than half its normal capacity
93
317613
6424
o coração dela havia enfraquecido
para menos da metade da capacidade normal
05:36
and had ballooned into
the distinctive shape of a takotsubo.
94
324061
4442
e inflado até se parecer
distintamente com um takotsubo.
05:40
But no other tests were amiss,
95
328527
2467
Mas nenhum outro exame
apresentou problemas;
05:43
no sign of clogged arteries anywhere.
96
331018
2435
não havia nenhum sinal
de artérias entupidas.
05:46
Two weeks later, her emotional state
had returned to normal
97
334400
4392
Duas semanas depois, o estado emocional
dela havia voltado ao normal
05:50
and so, an ultrasound confirmed,
98
338816
4223
e, como uma ultrassonografia confirmou,
05:55
had her heart.
99
343063
1286
o coração dela também.
05:56
Takotsubo cardiomyopathy has been linked
to many stressful situations,
100
344978
5929
A cardiomiopatia de takotsubo tem sido
associada a muitas situações estressantes,
06:02
including public speaking --
101
350931
1780
incluindo falar em público,
06:05
(Laughter)
102

2795
(Risos)
06:10
(Applause)
103
358772
4111
(Aplausos)
06:16
domestic disputes, gambling losses,
104
364952
3088
conflitos domésticos,
perdas em jogos de azar,
06:20
even a surprise birthday party.
105
368064
2141
até uma festa surpresa de aniversário.
06:22
(Laughter)
106
370229
1592
(Risos)
06:23
It's even been associated
with widespread social upheaval,
107
371845
4569
Também tem sido associada
a transtornos sociais generalizados,
06:28
such as after a natural disaster.
108
376438
2160
como após um desastre natural.
06:30
For example, in 2004,
109
378622
2306
Por exemplo, em 2004,
06:32
a massive earthquake devastated a district
on the largest island in Japan.
110
380952
5649
um forte terremoto devastou
um distrito na maior ilha do Japão.
06:39
More than 60 people were killed,
and thousands were injured.
111
387154
3718
Mais de 60 pessoas morreram
e milhares ficaram feridas.
06:43
On the heels of this catastrophe,
112
391422
2116
Pouco após a catástrofe,
06:45
researchers found that the incidents
of takotsubo cardiomyopathy
113
393562
5037
pesquisadores descobriram que a incidência
de cardiomiopatia de takotsubo
06:50
increased twenty-four-fold in the district
one month after the earthquake,
114
398623
5624
aumentou 24 vezes na região
um mês após o terremoto,
06:56
compared to a similar
period the year before.
115
404271
2701
comparada ao mesmo período
do ano anterior.
07:00
The residences of these cases
116
408131
2655
As residências dos afetados nesses casos
07:02
closely correlated with
the intensity of the tremor.
117
410810
2971
estavam estreitamente relacionadas
à intensidade do tremor.
07:05
In almost every case,
patients lived near the epicenter.
118
413805
4328
Em quase todos os casos,
os pacientes viviam perto do epicentro.
07:10
Interestingly, takotsubo cardiomyopathy
has been seen after a happy event, too,
119
418902
6211
Curiosamente, essa síndrome também
já foi observada após eventos felizes,
07:17
but the heart appears
to react differently,
120
425137
2706
mas o coração parece
reagir de forma diferente,
07:19
ballooning in the midportion,
for example, and not at the apex.
121
427867
3780
inflando na porção do meio,
por exemplo, não no ápice.
07:24
Why different emotional precipitants
would result in different cardiac changes
122
432382
5237
Por que causas emocionais diferentes
resultam em mudanças cardíacas diferentes
07:29
remains a mystery.
123
437643
1359
permanece um mistério.
07:31
But today, perhaps as an ode
to our ancient philosophers,
124
439567
4818
Mas, atualmente, talvez
em tributo aos antigos filósofos,
07:36
we can say that even if emotions
are not contained inside our hearts,
125
444409
6053
podemos dizer que, mesmo que as emoções
não estejam contidas no coração,
07:42
the emotional heart overlaps
126
450486
4999
o coração emocional coincide em parte
07:48
its biological counterpart,
127
456696
2277
com seu equivalente biológico,
07:50
in surprising and mysterious ways.

Heart syndromes, including sudden death,

vivenciando problemas emocionais intensos

08:18
in people who believed

que causaria sua morte
08:39
and against which
they were powerless to fight.

2387
e contra a qual não tinha como lutar.
08:41
This perceived lack of control,
Cannon postulated,
141
509920
3610
Essa aparente falta de controle,
afirmava Cannon,
08:45
resulted in an unmitigated
physiological response,
142
513554
3305
resultava em uma resposta
fisiológica intensa,
08:48
in which blood vessels
constricted to such a degree
143
516883
4605
na qual os vasos sanguíneos
se contraíam a tal ponto
08:53
that blood volume acutely dropped,
144
521512
3210
que o volume sanguíneo
caía de forma aguda,
08:56
blood pressure plummeted,
145
524746
1641
a pressão sanguínea despencava,
08:58
the heart acutely weakened,
146
526411
1577
o coração se extenuava
09:00
and massive organ damage resulted
from a lack of transported oxygen.
147
528012
4119
e a falta de transporte de oxigênio
resultava em danos fulminantes aos órgãos.
09:05
Cannon believed that voodoo deaths
148
533853
2267
Cannon acreditava que as mortes vudu
09:09
were limited to indigenous
or "primitive" people.
149
537017
3700
estavam limitadas a populações
autóctones ou "primitivas".
09:13
But over the years, these types of deaths
have been shown to occur
150
541511
4851
Mas, ao longo dos anos, mostrou-se
que esse tipo de morte também ocorre
09:18
in all manner of modern people, too.
151
546386
2679
em todo tipo de pessoas modernas.
09:21
Today, death by grief has been seen
in spouses and in siblings.
152
549642
6048
Atualmente, a morte por luto
tem sido observada em cônjuges e irmãos.
09:28
Broken hearts are literally
and figuratively deadly.
153
556217
4190
Corações partidos são mortais
literal e figurativamente.
09:32
These associations hold true
even for animals.
154
560855
2892
Essas associações são válidas
até para animais.
09:36
In a fascinating study in 1980
published in the journal "Science,"
155
564781
6146
Em um estudo fascinante em 1980,
publicado no periódico "Science",
09:42
researchers fed caged rabbits
a high-cholesterol diet
156
570951
3854
pesquisadores alimentaram coelhos em
gaiolas com uma dieta rica em colesterol
09:46
to study its effect
on cardiovascular disease.
157
574829
3188
para estudar seu efeito
em doenças cardiovasculares.
09:50
Surprisingly, they found that some rabbits
developed a lot more disease than others,
158
578624
5513
Descobriram com espanto que alguns coelhos
desenvolviam mais doenças do que outros,
09:56
but they couldn't explain why.
159
584161
1619
mas não sabiam explicar por quê.
09:57
The rabbits had very similar diet,
environment and genetic makeup.
160
585804
5755
Os coelhos tinham dieta, ambiente
e composição genética semelhantes.
10:03
They thought it might have
something to do with
161
591583
2620
Os pesquisadores acharam
que podia ter algo a ver
10:06
how frequently the technician
interacted with the rabbits.
162
594227
4171
com a frequência com que o técnico
interagia com os coelhos.
10:10
So they repeated the study,
163
598422
1767
Então repetiram o estudo,
10:12
dividing the rabbits into two groups.
164
600213
2357
dividindo os coelhos em dois grupos.
10:14
Both groups were fed
a high-cholesterol diet.
165
602594
2516
Os dois grupos recebiam
uma dieta rica em colesterol.
10:17
But in one group, the rabbits
were removed from their cages,
166
605880
4226
Mas, em um grupo, os coelhos
eram tirados de suas gaiolas
10:22
held, petted, talked to, played with,
167
610130
4300
e lhes davam colo e carinho,
além de conversarem e brincarem com eles,
10:26
and in the other group,
the rabbits remained in their cages
168
614454
2829
enquanto, no outro grupo, os coelhos
permaneciam nas gaiolas
10:29
and were left alone.
169
617307
1333
e eram deixados sozinhos.
10:31
At one year, on autopsy,
170
619249
3406
Após um ano, na autópsia,
10:34
the researchers found
that the rabbits in the first group,
171
622679
5494
os pesquisadores descobriram
que os coelhos do primeiro grupo,
10:40
that received human interaction,
172
628197
1939
que receberam interação humana,
10:42
had 60 percent less aortic disease
than rabbits in the other group,
173
630160
6180
tinham 60% menos doenças da aorta
do que os coelhos do outro grupo,
10:48
despite having similar cholesterol levels,
blood pressure and heart rate.
174
636364
5040
apesar dos níveis de colesterol, pressão
arterial e frequência cardíaca similares.
10:53
Today, the care of the heart has become
less the province of philosophers,
175
641704
6126
Atualmente, os cuidados com o coração
deixaram de ser uma área dos filósofos,
10:59
who dwell upon the heart's
metaphorical meanings,
176
647854
4950
que refletem sobre o significado
metafórico do coração,
11:04
and more the domain of doctors like me,
177
652828
3681
e se tornaram a esfera de médicos como eu,
11:08
wielding technologies
that even a century ago,
178
656533
2823
empregando tecnologias que,
até mesmo um século atrás,
11:11
because of the heart's exalted
status in human culture,
179
659380
3097
devido ao status elevado do coração
na cultura humana,
11:14
were considered taboo.
180
662501
1343
eram consideradas tabu.
11:16
In the process, the heart
has been transformed
181
664225
4235
Nesse processo, o coração foi transformado
11:20
from an almost supernatural object
imbued with metaphor and meaning
182
668484
5088
de objeto quase sobrenatural
impregnado de metáforas e significados
11:25
into a machine that can be
manipulated and controlled.
183
673596
4391
a uma máquina que pode ser
manipulada e controlada.
11:31
But this is the key point:
184
679040
2319
Mas esse é o elemento crucial:
11:33
these manipulations, we now understand,
185
681383
3161
percebemos agora que essas manipulações
11:36
must be complemented
by attention to the emotional life
186
684568
4918
devem ser acompanhadas
pela atenção à vida emocional
11:41
that the heart, for thousands of years,
was believed to contain.
187
689510
3242
que, por milhares de anos,
acreditou-se que o coração controlava.
11:45
Consider, for example,
the Lifestyle Heart Trial,
188
693836
3497
Considerem, por exemplo,
o artigo "Lifestyle Heart Trial",
11:49
published in the British journal
"The Lancet" in 1990.
189
697357
4504
publicado no periódico britânico
"The Lancet" em 1990.
11:53
Forty-eight patients with moderate
or severe coronary disease
190
701885
4043
Nele, 48 pacientes com doença
coronária moderada ou grave
11:57
were randomly assigned to usual care
191
705952
2743
receberam aleatoriamente
cuidados médicos tradicionais
12:00
or an intensive lifestyle
that included a low-fat vegetarian diet,
192
708719
5443
ou um estilo de vida intensivo que incluía
uma dieta vegetariana pobre em gorduras,
12:06
moderate aerobic exercise,
193
714186
1849
exercícios aeróbicos moderados,
12:08
group psychosocial support
194
716059
1571
apoio psicossocial em grupo
12:09
and stress management advice.
195
717654
1951
e orientação para controle do estresse.
12:11
The researchers found
that the lifestyle patients
196
719629
4746
Os pesquisadores descobriram que os
pacientes submetidos àquele estilo de vida
12:16
had a nearly five percent reduction
in coronary plaque.
197
724399
5134
tiveram redução de quase 5%
das placas coronarianas.
12:21
Control patients, on the other hand,
198
729557
2015
Os pacientes do grupo de controle, porém,
12:23
had five percent more
coronary plaque at one year
199
731596
4470
apresentaram 5% a mais
de placa coronariana após 1 ano
12:28
and 28 percent more at five years.
200
736090
2413
e 28% a mais após 5 anos.
12:30
They also had nearly double
the rate of cardiac events,
201
738527
4387
Também apresentaram quase o dobro
de eventos cardíacos,
12:34
like heart attacks,
coronary bypass surgery
202
742938
3379
como infartos, cirurgias
de revascularização do miocárdio
12:38
and cardiac-related deaths.
203
746341
1884
e mortes por causas cardíacas.
12:40
Now, here's an interesting fact:
204
748249
1575
E aqui temos um fato interessante:
12:42
some patients in the control group
adopted diet and exercise plans
205
750719
5294
alguns pacientes no grupo de controle
adotaram planos de dietas e exercícios
12:48
that were nearly as intense
as those in the intensive lifestyle group.
206
756037
4254
que eram quase tão intensos
quanto os do outro grupo.
12:53
Their heart disease still progressed.
207
761203
2086
Ainda assim, a doença cardíaca progrediu.
12:56
Diet and exercise alone were not enough
to facilitate coronary disease regression.
208
764900
5330
Só dieta e exercícios físicos não foram
o suficiente para o recuo da doença.
13:02
At both one- and five-year follow-ups,
209
770738
2804
No acompanhamento
após um ano e após cinco anos,
13:06
stress management
was more strongly correlated
210
774526
2969
o controle do estresse estava
mais estreitamente ligado
13:09
with reversal of coronary disease
211
777519
2052
com o recuo da doença coronariana
13:11
than exercise was.
212
779595
1528
do que o exercício físico.
13:14
No doubt, this and similar
studies are small,
213
782001
3617
Sem dúvida alguma, esse e outros estudos
semelhantes são pequenos
13:17
and, of course, correlation
does not prove causation.
214
785642
3181
e, é claro, correlação
não prova causalidade.
13:20
It's certainly possible that stress
leads to unhealthy habits,
215
788847
4358
Certamente, é possível que o estresse
leve a hábitos pouco saudáveis
13:25
and that's the real reason
for the increased cardiovascular risk.
216
793229
3279
e que essa seja a verdadeira razão
para o maior risco cardiovascular.
13:28
But as with the association
of smoking and lung cancer,
217
796532
3601
Mas, assim como na associação
entre o fumo e o câncer de pulmão,
13:32
when so many studies show the same thing,
218
800157
3257
quando tantos estudos
mostram a mesma coisa,
13:35
and when there are mechanisms
to explain a causal relationship,
219
803438
3410
e quando há mecanismos para explicar
uma relação de causalidade,
13:38
it seems capricious to deny
that one probably exists.
220
806872
4411
parece arbitrário negar
que ela provavelmente exista.
13:43
What many doctors have concluded
is what I, too, have learned
221
811798
3679
O que muitos médicos concluíram
é o que eu, também, descobri
13:47
in my nearly two decades
as a heart specialist:
222
815501
2658
em quase duas décadas
de trabalho como cardiologista:
13:51
the emotional heart intersects
with its biological counterpart
223
819033
4408
há uma intersecção do coração emocional
com sua contraparte biológica
13:55
in surprising and mysterious ways.
224
823465
2556
de formas surpreendentes e misteriosas.
13:58
And yet, medicine today continues
to conceptualize the heart as a machine.
225
826045
4778
Não obstante, a medicina atual continua
conceitualizando o coração como máquina.
14:03
This conceptualization
has had great benefits.
226
831312
3173
Essa definição já gerou
grandes benefícios.
14:07
Cardiology, my field,
227
835027
2558
A cardiologia, meu campo,
14:09
is undoubtedly one of the greatest
scientific success stories
228
837609
4638
é sem dúvida uma das maiores histórias
de sucesso científico
14:14
of the past 100 years.
229
842271
1587
dos últimos 100 anos.
14:17
Stents, pacemakers, defibrillators,
coronary bypass surgery,
230
845170
5648
Stents, marca-passos, desfibriladores,
revascularização do miocárdio,
14:22
heart transplants --
231
850842
1320
transplantes cardíacos,
14:24
all these things were developed
or invented after World War II.
232
852186
3949
tudo isso foi desenvolvido ou inventado
após a Segunda Guerra Mundial.
14:28
However, it's possible
233
856159
2111
Entretanto, é possível
14:30
that we are approaching the limits
of what scientific medicine can do
234
858294
5286
que estejamos nos aproximando dos limites
do que a medicina científica possa fazer
para combater as doenças cardíacas.
14:35
to combat heart disease.
235
863604
1598
14:37
Indeed, the rate of decline
of cardiovascular mortality
236
865226
3365
De fato, a taxa de queda
da mortalidade cardiovascular
14:40
has slowed significantly
in the past decade.
237
868615
3689
diminuiu consideravelmente
na última década.
14:45
We will need to shift to a new paradigm
238
873312
2980
Precisaremos adotar um novo paradigma
14:48
to continue to make the kind of progress
to which we have become accustomed.
239
876316
3995
para continuarmos a fazer o tipo
de progresso ao qual nos acostumamos.
14:52
In this paradigm, psychosocial factors
will need to be front and center
240
880335
5425
Nesse paradigma, fatores psicossociais
precisarão ser centrais
no modo de pensarmos
sobre problemas cardíacos.
14:57
in how we think about heart problems.
241
885784
2124
15:00
This is going to be an uphill battle,
242
888685
2138
Isso será uma tarefa difícil
15:02
and it remains a domain
that is largely unexplored.
243
890847
3943
e ainda é uma esfera pouco explorada.
15:07
The American Heart Association
still does not list emotional stress
244
895956
5165
A Associação Americana do Coração
continua não listando o estresse emocional
15:13
as a key modifiable risk factor
for heart disease,
245
901145
3791
como um fator modificável de risco
decisivo para doenças cardíacas,
15:16
perhaps in part because blood cholesterol
is so much easier to lower
246
904960
4803
talvez em parte porque seja muito mais
fácil diminuir o colesterol no sangue
15:21
than emotional and social disruption.
247
909787
2675
do que as interferências
emocionais ou sociais.
15:25
There is a better way, perhaps,
248
913582
2392
Talvez seja melhor
15:27
if we recognize that when
we say "a broken heart,"
249
915998
4877
reconhecermos que, quando falamos
em "coração partido",
15:32
we are indeed sometimes talking
about a real broken heart.
250
920899
4614
às vezes nos referimos
a um coração partido de verdade.
15:37
We must, must pay more attention to
the power and importance of the emotions
251
925537
6442
Precisamos, sim, prestar mais atenção
ao poder e à importância das emoções
15:44
in taking care of our hearts.
252
932003
1751
ao cuidarmos do nosso coração.
15:46
Emotional stress, I have learned,
253

Eu descobri que o estresse emocional
15:48
is often a matter of life and death.
254

é muitas vezes questão de vida ou morte.
